That’s a bad one. In 2008 I recall, a Kalitta Air 747 broke in two. I don’t recall if that followed a tail strike.

Same runway two days later:

A Lufthansa Airbus A380 carrying 351 passengers touched down too early at Munich Airport on Monday evening, damaging runway lights, the German airline confirmed.

That preliminary report really is a summary of the statements from the four pilots. It does not have information such as switch settings or other data recorder info. Given the length of the flight, there were two second shift pilots who were sitting in the cockpit for takeoff.
