On 2023-01-15, a Yeti Airlines ATR 72 twin turboprop airliner crashed on approach to Pohkara, Nepal, killing all on board. The preliminary accident report has now been released, and it is shocking: although aviation in mountainous Nepal is challenging, weather and terrain played no part in this accident, nor was there any mechanical problem with the airplane or involvement of its flight automation system. Two captains were at the controls, one a very senior check captain. And yet the plane crashed due to confusing two manual controls. This brings home just how easy it is to lose control of an airliner, and how difficult it is to recover once an upset occurs.
